DOCUMENTS All documentation for student-athletes is to be completed using the online database SportsWare Online.
No paper copies of these documents will be accepted
Completion of ALL documents via SportsWare is required as part of the student-athlete's medical eligibility
Failure to submit all information may delay clearance for participation in any sport

Incoming first-year and transfer student-athletes should carefully read through the welcome packet provided in the link below for all you need to know before you are cleared to compete including registering for SportsWare

RETURNING 
STUDENT-ATHLETES
Returning student-athletes should refer to the Returning Athletes section of the BCAT Paperwork. All returning student-athletes must update their insurance information and complete paperwork on SportsWare each year. Returning student-athletes are not required to submit new Pre-Participation Exams unless they have experienced a significant change in their health history (i.e. new medical condition, significant injury, etc)

Pre-Participation Exam
*All PPE forms must be signed by a physician within 12 months of the start of the current school year
ADD/ADHD Documentation 
*If a student-athlete is under a physician's care for ADD or ADHD and prescribed medication, this form is required by the NCAA. This form must be completed by the overseeing physician and returned to the athletic training department.
Sickle Cell Trait Policy and Information
Sickle Cell Test Form
*Effective January 2022, the NCAA is requiring all NCAA student-athletes to get a Sickle Cell Trait Solubility Test. A positive test is not grounds for exclusion from sport! More information can on the policy is included in the above link. The actual document/form can be found on the student-athlete's SportsWare account
 
VISITING TEAMS Locations: Flood Arena - VB, MBB, WBB -- Lower Level of Sports Center
Strong Stadium - FB, MSOC, WSOC, MLAX, WLAX, SB, BB, T/F -- North Entrance of Strong Stadium
Powerhouse -- Fitness Center on Third Floor

Coverage: Pre and Post Event Coverage
Athletic Trainers will be available 90 minutes prior to the start of an event in the training room and 30 minutes post-event

Supplies: Visitors should provide necessary taping supplies. 
Water bottles/cups, ice and bags, biohazard containment supplies, and emergency equipment will be available at each event on the sideline.

Therapeutic Modalities: Moist heat packs and ice are available without ATC prescription. Whirlpools (Flood Arena only), electrical stimulation and ultrasound available with ATC prescription
Beloit College should be notified of modality prescriptions 24 hours in advance
